1. Does your child attend Wyandotte Schools but live outside of Wyandotte? 

2. Does your child live outside of Wyandotte AND attend school outside of Wyandotte? 

3. Does your child live in Wyandotte, but go to school outside of Wyandotte Schools?

4. Has your child recently been registered with another organization within the DJFL?

If you answered YES to any of these questions, please print the Waiver Form and contact the Registrar for information
